72-year-old woman was seen in the office on 6-30-08. She is not having any vision change. She did have about 30-years ago an episode of vision change in her left eye associated with what she was told at the time was stress and that seems to have gotten better. You have been seeing her since 1984 and have noticed problems with the peripheral retina and suggested she come in here for retinal evaluation. Her vision is fine. She does have spots on her lungs, which she has been told are scar tissue. VISUAL ACUITY: Her vision is 20/30 OU. IOP: OD 15, OS 13. SLIT-LAMP EXAMINATION: There is trace nuclear sclerosis in both eyes. EXTENDED OPHTHALMOSCOPY: OD: Vertical C/D ratio is 0.2. There is no posterior vitreous separation. Superotemporal to the macula along the arcades there are a few elevated whitish, yellowish spots. OS: Vertical C/D ratio is 0.2. There is a posterior vitreous separation, again superotemporal to the macula just outside the arcades there are some large yellowish whitish spots. Photos confirm clinical findings. Ultrasound shows the spots to be calcified with shadowing posterior to them. IMPRESSION: 1. SCLEROCHOROIDAL CALCIFICATION IN BOTH EYES. 2. PROBABLE HISTORY OF CENTRAL SEROUS RETINOPATHY IN THE LEFT EYE. 3. CATARACT. DISCUSSION: I explained to the patient that she does have sclerochoroidal calcification. Recent studies have shown that about 50% of patients with this can have abnormalities of calcium and phosphorous metabolism and recommendation is that they be evaluated for possible Bartter and Gitelman syndrome.
