57-year-old woman was seen in the office on July 3, 2008. She has had blurred central vision in the right eye for about two to three weeks. The vision in the left eye is fine. VISUAL ACUITY: OD 20/70, OS 20/25. IOP: OD 14, OS 15. SLIT LAMP EXAM: Normal with clear lenses. EXTENDED OPHTHALMOSCOPY: OD: Vertical C/D ratio is 0.3. There is no posterior vitreous separation. There is a stage 2 macular hole about 200 microns across. OS: Vertical C/D ratio is 0.3. There is no posterior vitreous separation. The macula and periphery look healthy. OCT SCAN: The OCT scan of the right eye confirms the macular hole. The left retina is normal. IMPRESSION: 1. STAGE 2 MACULAR HOLE – RIGHT EYE DISCUSSION: I explained to the patient she does have a small stage 2 macular hole in the right eye. The chance of this spontaneously healing is about 20 percent. I suggested since there is no harm waiting a few months that she return for a check in six to eight weeks for a re-evaluation. If at that time the macular hole has not closed or if it has gotten larger, I would suggest we go ahead with vitrectomy. NOTE: patient refused surgery and further evaluations.
