53-year-old woman was seen in the office on 9/6/07. Also, thank you for your dictated letter, which was helpful and detailed. She was in for a comprehensive evaluation. She has amblyopia in the right eye, which sounds to be anisometropic amblyopia. She started wearing glasses when she was four. She did intermittent patching and the right eye sees okay but still not as good as the left eye. She has not noticed any vision change. You saw her and noticed a nevus in the back of the right eye and suggested she come in here for evaluation. VISUAL ACUITY: Vision OD is 20/30, OS is 20/16. IOP: OD 16, OS 17. AMSLER GRID: Normal OU. SLIT LAMP EXAM: Normal with clear lenses. EXTENDED OPHTHALMOSCOPY: OD: Vertical C/D ratio is 0.0. There is no posterior vitreous separation. There is a disc-diameter, flat, evenly pigmented choroidal nevus about 1000 microns inferior to the fovea. OS: Vertical C/D ratio is 0.0. There is no posterior vitreous separation. The macula and periphery look healthy. Photos confirm clinical findings. OCT SCAN: The OCT scan shows the nevus, but there is no intraretinal or subretinal fluid around it. IMPRESSION: 1. BENIGN CHOROIDAL NEVUS IN THE RIGHT EYE DISCUSSION: I explained to the patient she has a nevus in the right eye. The risk of it t ransforming to a malignancy is about 1:20,000. I asked her to return for check in six months, sooner should she notice any change or distortion in that eye, and I asked her to see you back regularly.
