88-year-old woman has noticed, especially over the last few weeks, difficulty reading and seeing headlines. She can only pick out one letter at a time now and then she has to reassemble the words. She has severe optic atrophy in the right eye from glaucoma. Both eyes have concentric geographic atrophy from dry age-related macular degeneration. She takes Timolol and Lumigan in the right eye. VISUAL ACUITY: Vision OD is 20/200, OS 20/100. IOP: OD 13, OS 14. There is a posterior chamber intraocular lens in good position in both eyes and the left eye has an open capsule. EXTENDED OPHTHALMOSCOPY: OD: Vertical C/D ratio is 0.9. The nerve is pale. There is geographic atrophy sparing the fovea. OS: Vertical C/D ratio is 0.8. The nerve is pale. There is geographic atrophy involving the fovea. OCT SCAN: The OCT shows retinal atrophy in both eyes. PHOTOGRAPHS: Photos confirm clinical findings. FLUORESCEIN ANGIOGRAPHY: The FA shows concentric geographic atrophy in both eyes just barely sparing about 300 microns of fovea in the left eye. IMPRESSION: 1. AGE-RELATED MACULAR DEGENERATION – BOTH EYES 2. CONCENTRIC GEOGRAPHIC ATROPHY – BOTH EYES 3. SEVERE OPTIC ATROPHY FROM END STAGE GLAUCOMA – RIGHT EYE DISCUSSION: I explained to the patient the macula in both eyes has dry age-related macular degeneration with geographic atrophy in the left eye, which is her better eye, which has less advanced glaucoma but unfortunately now has foveal involvement causing vision loss. At this moment there is no treatment for that. I asked her to continue taking her fish oils and supplements. I suggested she see you for comprehensive evaluation to make sure her prescription is optimized. In addition, I suggested she use a good reading light and avoid significant magnification which can sometimes be detrimental in cases such as hers. She will return in six months.
