80 year old woman with decreased vision and blurring in the left eye for the past year. She reports that she has blurring that comes and goes and floaters that bother her when she reads. The patient reports that the floaters that bother her have been occurring on a daily basis lately. She reports that she is having a “hard time reading”. She denies any ophthalmic pain and denies similar symptoms in the right eye. Visual acuity with correction 20/30-3 OD and 20/30-2 OS. She reads with correction J3 OU. IOP: 16 OD and 15 OS. IMPRESSION: 1. ASTEROID HYALOSIS WITH SUBJECTIVE AND OBJECTIVE VISUAL SIGNIFICANCE – LEFT EYE 2. INTERMEDIATE DRY MACULAR DEGENERATION 3. POSTERIOR VITREOUS SEPARATION – RIGHT EYE, LIKELY PRESENT LEFT EYE AS WELL 4. STABLE PSEUDOPHAKIA – BOTH EYES We talked about the option of observation versus the observation of vitrectomy. I talked about the risks and benefits of surgery and I explained that vitrectomy for vitreous opacities is a relatively safe procedure. We talked about how there is a possibility of losing the eye to complications of surgery and how it would be best if she could cease her anticoagulation prior to vitrectomy. We talked about the three to five percent risk of retinal detachment associated with uncomplicated vitrectomy.
