Initial consult: He bent down to pick up a Frisbee yesterday and a branch hit his left eye. He had a scleral laceration. He underwent repair which included a patch graft with pericardium as you had noted the scleral laceration did extend posteiorly by 2.5 mm from the limbus. He denies any injury to the right eye. He denies any significant ocular pain OS but feels his eye is “swollen”. He feels the vision has improved and he “sees colors now”. He is on no topical medications currently. February 22, 2010. This pleasant 11-year-old had a choroidal rupture in the left eye with a macular hemorrhage from an injury on the 19th of November. His vision seems about the same to him. VISUAL ACUITY: OS 20/200. IOP: 12. The lens is clear. EXTENDED OPHTHALMOSCOPY: OS: Vertical C/D ratio is 0.2. The hemorrhage is now retracted. It is just nasal to the center of the macula. The choroidal rupture on the temporal edge I can now see clearly and it looks to spare the fovea. Photos confirm clinical findings. IMPRESSION: 1. CHOROIDAL RUPTURE – LEFT EYE 2. REABSORBING SUB MACULAR HEMORRHAGE – LEFT EYE DISCUSSION: I explained to the patient that the blood looks to have retracted now away from the center of the fovea and I do see a crisp foveal reflex there, which hopefully will be indicative of his vision ultimately improving. I asked him to return in two months or sooner if he should notice a problem.
