75-year-old woman was seen in the office on September 9, 2009. She saw you a year ago and then in a recent exam it was noticed her vision in the left eye had substantially declined. She is not clear as to when that exactly happened. She had not noticed a vision change in the left eye. Her right eye sees pretty well. VISUAL ACUITY: OD 20/30, OS 20/400. IOP: OD 10, OS 7. SLIT EXAMINATION: There is corneal verticillata in both eyes from her Amiodarone and there is 4+ nuclear sclerosis in both eyes. EXTENDED OPHTHALMOSCOPY: OD: Vertical C/D ratio is 0.3. There is no posterior vitreous separation. The macula and periphery look healthy. OS: Vertical C/D ratio is 0.3. There is no posterior vitreous separation. There is a 500 micron macular hole. OCT SCAN: The OCT scan shows in the right eye a Stage Zero macular hole with vitreomacular traction. The left eye has a Stage 3 macular hole. IMPRESSION: 1. STAGE 3 MACULAR HOLE – LEFT EYE 2. STAGE ZERO MACULAR HOLE – RIGHT EYE 3. CATARACTS 4. CORNEAL VERTICILLATA DISCUSSION: I explained to the patient that she does have a macular hole in the left eye. With surgery there is at least a ninety five percent chance of closing the hole and a chance of substantially improving her vision. Given the thickness of her cataract though and the possible necessity to peel internal membrane, I suggest that she first have her cataract removed.
