87 Year old woman with dry age-related macular degeneration in both eyes. She takes the ICAPS. She follows her vision and follows the grid. With both eyes open she is fine, but when she closes the left eye, things are wavier. VISUAL ACUITY: Vision OD is 20/100, OS is 20/40. IOP: OD 18, OS 20. The posterior chamber intraocular lens is in good position in both eyes. EXTENDED OPHTHALMOSCOPY: OD: Vertical C/D ratio is 0.0. There is a posterior vitreous separation. There is nonfoveal geographic atrophy just inferior to the fovea which touches the fovea. The lens has patchy calcified drusen. OS: Vertical C/D ratio is 0.0. There is a posterior vitreous separation. There is nonfoveal geographic atrophy inferior and superior to the fovea within 500 microns of the center and there is patchy drusen. OCT SCAN: The OCT scan shows the macula to be dry but atrophic. PHOTOGRAPHS: Photos confirm clinical findings. FLUORESCEIN ANGIOGRAPHY: There is patchy hyperfluorescence in the macula, but there is no evidence of any neovascularization. IMPRESSION: 1. DRY AGE-RELATED MACULAR DEGENERATION – BOTH EYES 2. FOVEAL-TOUCHING GEOGRAPHIC ATROPHY – RIGHT EYE DISCUSSION: I explained to the patient she does have geographic atrophy in the right eye adversely affecting the center of the fovea and that is making her vision in the right eye worse. Unfortunately, at this moment we have no treatment for that. I did ask her to see you back periodically. She will continue taking her eye vitamins.
