Traction Retinal Detachment Nasal Fibrosis

33-year-old woman has proliferative diabetic retinopathy in the left eye with fibrosis and tractional retinal detachment. Her vision is hazy in that eye. I saw her a few days ago but she could not stay for treatment. She returns now for evaluation and probable therapy.
VISUAL ACUITY: Vision OD is 20/30, OS 20/120. IOP: OD 15, OS 14. The posterior chamber intraocular lens is in good position in both eyes with 1+ posterior capsular opacity.
EXTENDED OPHTHALMOSCOPY:
OD: Vertical C/D ratio is 0.2. There are patchy retinal hemorrhages.
OS: Vertical C/D ratio is 0.2. There is fibrotic tractional retinal detachment.
PHOTOGRAPHS: Photos confirm clinical findings.
FLUORESCEIN ANGIOGRAPHY: FA shows multiple microaneurysms in the right eye. The left eye has nonperfusion in the far periphery.
IMPRESSION: 1. PROLIFERATIVE DIABETIC RETINOPATHY – LEFT EYE
2. PRERETINAL FIBROSIS – LEFT EYE
3. BACKGROUND DIABETIC RETINOPATHY – RIGHT EYE
DISCUSSION: I explained to the patient with panretinal laser we can reduce the risk of vision loss in the left eye. I treated her with panretinal laser today without complication.
She will return for check in a few weeks for further therapy and then ultimately vitrectomy might be necessary. She has been checked and she does not have sickle cell traits.
