56-year-old woman was seen in the office on 8/26/2011. She was in for a comprehensive evaluation. Her vision is fine. You saw a spot on the retina of the left eye and suggested she come here for an evaluation. I appreciate your note, which was helpful, with the drawing showing where the spot was. VISUAL ACUITY: Vision OD is 20/20, OS is 20/16. IOP: OD 15, OS 18. AMSLER GRID: The Amsler grid is normal. COLOR VISION: The color vision is normal. SLIT LAMP EXAM: There is trace nuclear sclerosis in both eyes. EXTENDED OPHTHALMOSCOPY: OD: Vertical C/D ratio is 0.2. There is no posterior vitreous separation. The macula and periphery look healthy. OS: Vertical C/D ratio is 0.2. There is a disc area hypopigmented lesion just 3-disc diameters inferior to the optic nerve with sharp, even borders. The smooth retinal vessels over the surface are intact. PHOTOGRAPHS: Photos confirm clinical findings. IMPRESSION: 1. CONGENITAL HYPERTROPHY OF THE RETINAL PIGMENT EPITHELIUM – LEFT EYE DISCUSSION: I explained to the patient she has a small patch of congenital hypertrophy of the retinal pigment epithelium in the left eye which is equivalent to a birthmark. These are benign and they never transform into tumors. It is in an area where you sometimes see colobomas; however, given the intact retinal vessels over the surface of it, that rules out the possibility of it being a coloboma.
