31-year-old man had an episode of toxoplasmosis in the left eye, where unfortunately it adversely affected his central vision that was back in 2004 (7 years ago) I have photos from then. It looked like it was a primary toxoplasmosis lesion in the center of the macula in the left eye. Unfortunately, he was treated and the spot dried up, but then he did notice recently a change in his visual function. He plays softball, he was getting hit by the ball and having trouble seeing and he said that is how he noticed the toxoplasmosis when it first occurred and he was concerned it might have recurred. VISUAL ACUITY: OD 20/16, OS 20/80. IOP: OD 11, OS 10. Color vision is normal in both eyes. SLIT EXAMINATION: Normal with clear lenses. EXTENDED OPHTHALMOSCOPY: OD: Vertical C/D ratio is 0.2. There is no posterior vitreous separation. The macula and periphery look healthy. OS: Vertical C/D ratio is 0.2. There is a partial posterior vitreous separation. There are trace vitreous cells, which look to be old. There is a plaque chorioretinal scar just on the superior edge of the fovea as well as a tiny scar superonasal to that. OCT SCAN: The OCT scan shows retinal atrophy in the left eye. The right eye is normal. Photos confirm clinical findings. FLUORESCEIN ANGIOGRAPHY: Fluorescein angiography shows predominately hypofluorescence where the toxoplasmosis scar is centrally with some hyperfluorescence around the edge, which fades in the late frames. IMPRESSION: 1. MACULAR SCAR – LEFT EYE 2. HISTORY OF TOXOPLASMOSIS DISCUSSION: I explained to the patient unfortunately the macular scar in the left eye is threatening his fovea and those scars do sometimes grow by ten to twenty percent over the course of a lifetime and it may be that it is starting to affect his visual function. I asked him to see you back for a comprehensive evaluation. He does need to start wearing protective glasses to protect his right eye. I asked him to return here for a check in two months, but I told him if he notices no change, he can cancel that appointment.
