Adult Vitelliform Pattern Dystrophy - 60 Year Old Woman
|
|
60-year-old woman has pattern dystrophy in both eyes for some time. She was last seen here a year ago. She notices, if anything, her vision recently is improving. She is noticing less distortion and less blind spots.
VISUAL ACUITY: Vision OD is 20/20, OS is 20/40. IOP: 15 in both eyes. The lenses are clear.
EXTENDED OPHTHALMOSCOPY:
OD: Vertical C/D ratio is 0.3. There is no posterior vitreous separation. There is a yellow pigment spot under the fovea, about 500 microns across.
OS: Vertical C/D ratio is 0.3. There is no posterior vitreous separation. There is a yellow pigment spot under the fovea, about 500 microns across.
OCT SCAN: The OCT scan in the right eye shows pigment epithelial thickening centrally with overlying foveal atrophy. The left eye OCT scan shows a gap in the photoreceptors centrally. These are largely unchanged from the previous scans.
IMPRESSION:
1. PATTERN DYSTROPHY-TYPE DRY MACULAR DEGENERATION – BOTH EYES
DISCUSSION: I explained to the patient the maculae look stable, but I am encouraged her visual function has improved, and it is possible her tissue is healing itself. Most patients with pattern dystrophy do have slow vision loss, if any at all.
