85-year-old man has wet age-related macular degeneration in the left eye for which he had injections of Lucentis from January of 2010 to March of 2011. The vision is decent in that eye and the right eye continues to see fine. VISUAL ACUITY: Vision OD is 20/30, OS is 20/60. IOP: OD 13, OS 17. There is a ReSTOR posterior chamber intraocular lens in good position in both eyes. EXTENDED OPHTHALMOSCOPY: OD: Vertical C/D ratio is 0.5. The macula is dry with some hyperpigmented spots superior to the macula. OS: Vertical C/D ratio is 0.6. The macula is dry with minimal fibrosis superotemporal to the fovea. OCT SCAN: The OCT scan confirms an absence of intraretinal or subretinal fluid in either eye. There is minimal retinal atrophy in the left eye. PHOTOGRAPHS: Photos confirm clinical findings. FLUORESCEIN ANGIOGRAPHY: The FA shows some hyperpigmented spots superior to the macula in the right eye. There is no leakage though, just staining. The left eye similarly has an area of hyperfluorescence just superior to the fovea which stains in the late frames but does not leak. IMPRESSION: 1. WET AGE-RELATED MACULAR DEGENERATION – LEFT EYE 2. INVOLUTED SUBFOVEAL CHOROIDAL NEOVASCULAR MEMBRANE – LEFT EYE 3. SUBRETINAL CHOROIDAL NEOVASCULAR MEMBRANE – LEFT EYE 4. DRY MACULAR DEGENERATION WITH HYPERPIGMENTED SPOTS – RIGHT EYE DISCUSSION: I explained to the patient the maculae at the moment are both dry. I asked him to follow his vision, follow the grid, and to return urgently if he should notice a change in either; otherwise, he will return for check in six months
