93-year-old man has wet age-related macular degeneration in his left eye with a subfoveal pigment epithelial detachment. His macula is progressing despite treatments, so I tightened up his treatment interval. With every two-to-three week injections, his vision remains reasonable in that eye. The vision seems to possibly be a little worse. He is taking a glaucoma drop in the left eye. VISUAL ACUITY: Vision OS is 20/40. IOP: OS 15. There is a posterior chamber intraocular lens in good position. EXTENDED OPHTHALMOSCOPY: OS: Vertical C/D ratio is 0.2. There is a pigment epithelial detachment centrally. There are striking radiating chorioretinal folds emanating off of the scar, which suggests it is contracting. OCT SCAN: The OCT scan shows macular edema. IMPRESSION: 1. WET AGE-RELATED MACULAR DEGENERATION – LEFT EYE 2. MACULAR EDEMA – LEFT EYE 3. PIGMENT EPITHELIAL DETACHMENT – LEFT EYE 4. MACULAR PUCKER 5. RADIATING CHORIORETINAL FOLDS
