54-year-old woman has had a visually significant and debilitating vitreous opacity in the right eye, which has been there for 2 years. It is, if anything, getting worse. When she moves her eye around it lands right in her central vision most of the time. VISUAL ACUITY: Her vision OD is 20/20, OS is 20/80. IOP: OD 16, OS 15. The lenses are clear. EXTENDED OPHTHALMOSCOPY: OD: Vertical C/D ratio is 0.0. There is a dense vitreous opacity in her macula. There is a choroidal nevus superiorly. OS: Vertical C/D ratio is 0.0. The macula and periphery are fine. Photos confirm clinical findings. IMPRESSION: 1. DENSE, CHRONIC, SYMPTOMATICI VITREOUS OPACITY – RIGHIT EYE DISCUSSION: I explained to the patient that given the adverse affect the vitreous opacity has in her visual function, she is an excellent candidate for vitrectomy surgery, which will remove the opacity, with only about a 1 in 1000 risk of infection, and about a 2% risk of retinal detachment. I scheduled her for surgery within the next few weeks.
