65-year-old woman had chronic endophthalmitis in the left eye, possibly from fungus. She had a vitrectomy for this on August 20th and returns today, the day after surgery. At the time of the vitrectomy, she did have multiple white spots in the inferior retina, suggestive of a fungal vitreoretinitis. VISUAL ACUITY: OS 20/400. IOP: OS 13. There is 3+ conjunctival injection. The cornea is clear. The posterior chamber intraocular lens is in good position. EXTENDED OPHTHALMOSCOPY: OS: Vertical C/D ratio is 0.2. There are 2+ vitreous cells. There are white spots in the inferior retina. Photos confirm clinical findings. IMPRESSION: 1. PROBABLE FUNGAL ENDOPHTHALMITIS – LEFT EYE DISCUSSION: I explained to the patient we are still waiting for the culture results on the endophthalmitis. Cultures were negative but stains showed fungal elements. She ultimately lost vision from a complex retinal detachment and is attached now under oil with ambulatory (5/200) vision.
