10-year-old child has juvenile central retinoschisis in both eyes. His vision, unfortunately, is declining since I saw him last a little over a year ago. VISUAL ACUITY: Vision OD is 20/60, OS is 20/100. IOP: OD 14, OS 13. The lenses are clear. EXTENDED OPHTHALMOSCOPY: OU: Vertical C/D ratio is 0.1. There is both foveal and peripheral retinoschisis. OCT SCAN: There is retinoschisis with a central foveal thickness increased from last visit. The right eye now is at 429 microns and the left eye 391 microns. IMPRESSION: 1. JUVENILE X-LINKED RETINOSCHISIS – BOTH EYES 2. INCREASED FOVEAL THICKENING WITH DECREASING VISION – BOTH EYES DISCUSSION: I explained to the patient and his mother there are some recent studies suggesting systemic and topical carbonic anhydrase inhibitors can reduce the retinal thickness and improve the visual acuity in patients with X-linked juvenile retinoschisis. Since the topical drops have fewer systemic side effects, I started him on Trusopt drops three times daily, and I asked him to return for check in three months to see how the eyes are doing. If he does not have any improvement, then I will discuss with you the possibility of putting him on oral acetazolamide. Follow-up 6 months: 11-year-old child has x-link juvenile retinoschisis. He is on Trusopt drops three times daily since June of 2008. His vision seems to be holding stable. VISUAL ACUITY: Vision OU is 20/60. IOP: OD 14, OS 13. The lenses are clear. There are foveal cysts. There is peripheral retinoschisis. OCT SCAN: There is a central foveal thickness in the right eye of 319 microns and the left eye of 339 microns. This is substantially better than a year ago when the right was 429 and the left eye 391 microns. IMPRESSION: 1. DECREASED FOVEAL CYSTS – BOTH EYES 2. X-LINK JUVENILE RETINOSCHISIS – BOTH EYES DISCUSSION: I explained to the patient the maculae look substantially better today and his vision has improved, especially in his left eye. I am hopeful the drops will help stabilize the vision. I suggested he take them twice a day and return for check in six months.
