85-year-old woman has age-related macular degeneration in both eyes with geographic atrophy. She has noticed her vision getting slightly worse since I saw her last in December. VISUAL ACUITY: OD 20/70, OS 20/400. IOP: OD 12, OS 17. She has low filtering blebs in both eyes. There is a posterior chamber intraocular lens is in good position in both eyes. EXTENDED OPHTHALMOSCOPY: OD: Vertical C/D ratio is 0.5. There is geographic atrophy. There is no intraretinal or subretinal fluid. OS: Vertical C/D ratio is 0.5. There is geographic atrophy. There is no intraretinal or subretinal fluid. OCT SCAN: The OCT scan show retinal atrophy. Photos confirm clinical findings. FLUORESCEIN ANGIOGRAPHY: Fluorescein angiography shows concentric geographic atrophy, predominately nasal to the fovea in each eye. The geographic atrophy is unfortunately now closer to the foveal center. IMPRESSION: 1. AGE-RELATED MACULAR DEGENERATION – BOTH EYES 2. PROGRESSING GEOGRAPHIC ATROPHY – BOTH EYES 3. SUBSTANTIAL OBJECTIVE VISION LOSS 4. GLAUCOMA WITH FILTERING BLEBS 5. DIABETES DISCUSSION: I explained to the patient that the macular degeneration does look slightly worse and although she only notices a slight decline in vision, her vision in fact measures substantially worse than it did when she was last here. When I saw her in December she was seeing 20/40 out of both eyes and now she is down to 20/70 and 20/400. Sometimes patients with geographic atrophy can develop seemingly sudden vision loss when the geographic atrophy progressed to involve the center of the macula.
