55-year-old woman has multifocal choroiditis in both eyes because she was HLA A-29 positive. She has presumptive birdshot chorioretinopathy. She has not noticed any vision changes. You saw a problem with the macula in the right eye and suggested she come in here for evaluation. She is diabetic and her blood pressures have recently been running a little high. VISUAL ACUITY: OD 20/40, OS 20/20. IOP: OD 11, OS 14. The posterior chamber intraocular lens is in good position in both eyes. The left eye has 1+ posterior capsular opacity. EXTENDED OPHTHALMOSCOPY: OD: Vertical C/D ratio is 0.8. There is a retinal arterial macroaneurysm just temporal to the optic nerve with a ring of exudate around it, just stopping short of the fovea. There is patchy chorioretinal scarring. OS: Vertical C/D ratio is 0.4. There is patchy chorioretinal scarring. OCT SCAN: The right eye does show macular edema nasal to the fovea. The left eye shows patchy atrophy. Photos confirm clinical findings. IMPRESSION: 1. Retinal Arterial MACROANEURYSM WITH MACULAR EDEMA – RIGHT EYE 2. HLA A-29 POSITIVE BIRDSHOT RETINAL CHORIODITIS DISCUSSION: I explained to the patient there is a leaky blood vessel in the right eye which is threatening the fovea, but not at this moment at all in the fovea. I suggested we leave the eye alone. I did, however, ask her to return for a check in three months, sooner should she notice any problem. She will see you periodically.
