85-year-old woman has felt sick and weak for about the last two months. She is having headaches, jaw claudication, and her eyes ache. Then, three days ago she had sudden vision loss in the left eye. You saw her with optic nerve swelling and suggested she come in here for evaluation. She had coronary bypass surgery in 2004. VISUAL ACUITY: Vision OD is 20/80, OS is light perception. IOP: OD 13, OS 15. The posterior chamber intraocular lens is in good position. EXTENDED OPHTHALMOSCOPY: OD: Vertical C/D ratio is 0.1. The nerve has pallor of the superior pole of the optic nerve and edema. OS: Vertical C/D ratio is 0.0. There is 360 degrees of swelling and pallor of the optic nerve. PHOTOGRAPHS: Photos confirm clinical findings. IMPRESSION: 1. ISCHEMIC OPTIC NEUROPATHY – BOTH EYES 2. PROBABLE GIANT CELL ARTERITIS DISCUSSION: I explained to the patient, given her constitutional symptoms and the pallor and swelling of the optic nerves, she probably has giant cell arteritis. I am going to obtain a sedimentation rate and C-reactive protein today and then start her on Prednisode 60 mg today and then obtain a temporal artery biopsy tomorrow. She is already taking an aspirin daily, and I suggested she continue that, although I asked her to not take one tomorrow morning. Thank you for allowing me to participate in her care.
