Chronic Retinal Detachment with New Foveal Involvement

25-year-old man was seen in the office on 3/29/2011. He has had vision loss in the left eye for a month. He noticed a shadow up above, which went through his central vision. He has not had any injuries in the left eye. The vision in the right eye is fine.
VISUAL ACUITY: Vision OD is 20/20, OS is 20/400. IOP: OD 14, OS 13.
SLIT LAMP EXAM: Normal with clear lenses.
EXTENDED OPHTHALMOSCOPY:
OD: Vertical C/D ratio is 0.1. The macula and periphery look healthy.
OS: Vertical C/D ratio is 0.1. There is an inferotemporal retinal detachment extending from 3:00 o’clock around to 8:00 o’clock with a retinal dialysis between 5:00 and 6:00 o’clock. The macula is detached. It looks like there is an epiretinal membrane. There are also bands of subretinal fibrosis inferiorly showing previous demarcation lines, suggesting detachments at least three months old and possibly longer.
IMPRESSION:
1. MACULA-OFF RETINAL DETACHMENT – LEFT EYE
DISCUSSION: I explained to the patient it looks like the retinal detachment has been there for awhile and it looks like it is of traumatic etiology. He denies any history of trauma though and just noticed it a month ago. I scheduled him for a scleral buckle next week, which has about a 90% chance of reattaching the retina.
