Acute Zonal Occult Outer Retinopathy - AZOOR - Pseudoretinitis Pigmentosa

65-year-old woman has pseudoretinitis pigmentosa in the right eye only, most likely from acute zonal occult outer retinopathy. Her vision is stable and she doesn’t notice the floaters as much as she did previously when I saw her five years ago.
VISUAL ACUITY: OD 20/25, OS 20/25. IOP: OD 13, OS 15.
SLIT EXAMINATION: There is 1+ nuclear sclerosis in both eyes.
EXTENDED OPHTHALMOSCOPY:
OD: Vertical C/D ratio is 0.3. There is pseudoretinitis pigmentosa extending inferior off the nerve and then into the temporal periphery, but sparing the macula. There is also 2+ asteroid hyalosis.
OS: Vertical C/D ratio is 0.2. There is no posterior vitreous separation and the macula and periphery look healthy.

IMPRESSION:
1. ACUTE ZONAL OCCULT OUTER RETINOPATHY – RIGHT EYE
DISCUSSION: I explained to the patient the areas of pseudoretinitis pigmentosa are unchanged. This is in all likelihood from an acute zonal occult outer retinopathy. It is remotely possible this could have been from an optic nerve to retinal detachment, but I don’t see any optic nerve pit present at the moment. Sometimes they will close and the area can sometimes look more normal over time. The reason I say that is because the pit pseudoretinitis pigmentosa does connect to the optic nerve in a way that looks like you would see something dependant from gravity, although most optic nerve detachments seem to involve the macula.
