55-year-old woman has no noticed in the right eye a few months ago flashing lights and those have since went away and then the left eye two weeks ago has new flashes and cobwebs. The flashes stopped about ten days ago. The cobwebs are slightly temporal. VISUAL ACUITY: OD 20/30, OS 20/25. IOP: OD 10, OS 12. SLIT EXAMINATION: There is 1+ nuclear sclerosis in both eyes. EXTENDED OPHTHALMOSCOPY: OD: Vertical C/D ratio is 0.2. There is posterior vitreous separation and 1+ macular drusen. OS: Vertical C/D ratio is 0.2. There is posterior vitreous separation and 1+ macular drusen with scleral depression and no tears. There was a 4 disc area of congenital hypertrophy of the retinal pigment epithelial lesion superotemporally at the equator at 1 o’clock. Photos confirm clinical findings. IMPRESSION: 1. ACUTE POSTERIOR VITREOUS SEPARATION – LEFT EYE WITHOUT TEARS 2. VITREOUS SEPARATION – RIGHT EYE 3. CONGENITAL HYPERTROPHY OF THE RETINAL PIGMENT EPITHELIUM – LEFT EYE DISCUSSION: I explained to the patient the left eye does have a birth mark in it. I don’t see anything there that looks sinister and the risk of retinal tear or retinal detachment in that eye is about two percent.
