80-year-old woman has a retinal arterial macroaneurysm in the right eye I saw two years ago. She noticed the sudden onset of vision loss last night at 8:00. VISUAL ACUITY: Vision OD is 5/200. IOP: OD 12. The posterior chamber intraocular lens is in good position. There is a 2+ vitreous hemorrhage centered around the macroaneurysm. IMPRESSION: 1. RUPTURED RETINAL ARTERIAL MACROANEURYSM – RIGHT EYE 2. VITREOUS HEMORRHAGE – RIGHT EYE 3. RETINAL HEMORRHAGE – RIGHT EYE DISCUSSION: I explained to the patient that the blood that is in the eye should absorb over the next 3-6 months. I did ask her to return for check in a month to see how she is doing. Photo before rupture from 2 years previous with VA 20/25.
