Ocular Ischemic Syndrome - Microaneurysms right eye (fellow eye old CRAO)

66-year-old woman lost vision in the left eye from a central retinal artery occlusion. Vision OD is fine.
VISUAL ACUITY: OD 20/25, OS 20/50. IOP: OD 17, OS 19. There is 2+ nuclear sclerosis in both eyes.
EXTENDED OPHTHALMOSCOPY:
OD: Vertical C/D ratio is 0.1. There are patchy hemorrhages inferotemporal and superotemporal to the fovea.
OS: Vertical C/D ratio is 0.5. The nerve is pale. The macula is dry and atrophic.
PHOTOGRAPHS: Photos confirm clinical findings.
FLUORESCEIN ANGIOGRAPHY: The FA shows clustered microaneurysms temporal to the fovea in the right eye which leak in the late frames. The left eye is angiographically normal.
IMPRESSION:
1. OCULAR ISCHEMIC SYNDROME WITH RETINAL ARTERIAL MICROANEURYSMS – RIGHT EYE
2. HISTORY OF CENTRAL RETINAL OCCLUSION – LEFT EYE
DISCUSSION: I explained to the patient unfortunately the right eye looks like it is manifesting ocular ischemic syndrome, which is troublesome because that is her better eye.
