81-year-old man has age-related macular degeneration in both eyes. He had intravitreal injections of Avastin between August of 2009 and September of 2010. The vision in that eye remains about stable. His right eye continues to see fine. VISUAL ACUITY: Vision OD is 20/40, OS is 20/80. IOP: OD 15, OS 13. The posterior chamber intraocular lens is in good position in both eyes. The right eye has a 3+ posterior capsular opacity which is approaching the visual axis. The left eye has an open capsule. EXTENDED OPHTHALMOSCOPY: OD: Vertical C/D ratio is 0.5. There are patchy pigment spots predominantly temporal to the fovea. There is nonfoveal geographic atrophy mostly temporally. There is no subretinal fluid, hemorrhage, or exudate. OS: Vertical C/D ratio is 0.5. Again, there is nonfoveal geographic atrophy inferotemporal and superotemporal to the fovea with macular drusen. OCT SCAN: The OCT scan shows no intraretinal or subretinal fluid in either eye. FLUORESCEIN ANGIOGRAM: FA shows patchy hyperfluorescence corresponding to the nonfoveal geographic atrophy which fades in the late frames. IMPRESSION: 1. AGE-RELATED MACULAR DEGENERATION – BOTH EYES 2. NONFOVEAL GEOGRAPHIC ATROPHY – BOTH EYES 3. INVOLUTED SUBFOVEAL CHOROIDAL NEOVASCULAR MEMBRANE – LEFT EYE DISCUSSION: I explained to the patient the maculae look stable and dry. There is still a risk of recurrent neovascularization in the left eye but it probably does decline with time. I asked him to return here in four months, sooner should he notice a problem. He will see you back periodically. Thank you for allowing me to participate in his care.
