Retinal Detachment and Macular Hole

pleasant 51-year-old woman was seen in the office on 8-9-07. About 3 or 4 months ago she noticed some flashing lights. Her vision became hazy and dark, especially when she was covering her left eye. She also had flashing lights and some haziness with her eyes closed.
VISUAL ACUITY: OD 2/200, OS 20/20. IOP: OD 13, OS 10.
SLIT-LAMP EXAMINATION: There is 1+ nuclear sclerosis in the right eye, trace nuclear sclerosis in the left eye.
EXTENDED OPHTHALMOSCOPY:
OD: Vertical C/D ratio is 0.3. There is a posterior vitreous separation. There is a macula-off retinal detachment with a macular hole. There is a large retinal tear at 12:00 with lattice degeneration and a moderate retinal tear at 10:00. There is lattice degeneration also in the periphery at 6:00, 7:00 and 8:00.
OS: Vertical C/D ratio is 0.3. There is no posterior vitreous separation. There is lattice degeneration from 12:00 to 6:00.
IMPRESSION:
1. MACULA-OFF RETINAL DETACHMENT IN THE RIGHT EYE.
2. MACULAR HOLE IN THE RIGHT EYE.
3. LATTICE DEGENERATION IN BOTH EYES.
DISCUSSION: I explained to the patient given the configuration of the retinal detachment, especially with the macular hole that we can do a scleral buckle and then a vitrectomy with gas and laser
