69-year-old woman was seen in the office on 2/3/2012. She said her biological mother had a lazy eye and she notices she has some difficulty using her two eyes together. She was in for a comprehensive evaluation and a pair of glasses, at which time you saw a lesion in the back of the left eye and suggested she come here for an evaluation. VISUAL ACUITY: Vision OD is 20/30, OS is 20/40. IOP: 17 in both eyes. There is 2+ nuclear sclerosis in both eyes. EXTENDED OPHTHALMOSCOPY: OD: Vertical C/D ratio is 0.2. There is a partial posterior vitreous separation. The macula and periphery look healthy. OS: Vertical C/D ratio is 0.2. There is a partial posterior vitreous separation in the superonasal periphery. There is an area of myelinated nerve fiber layer extending from about 9:00 o’clock up to 10:00 o’clock and starting a disc diameter off the optic nerve. IMPRESSION: 1. CONGENITAL MYELINATED NERVE FIBER LAYER – LEFT EYE 2. CATARACTS – BOTH EYES
