65-year-old man was seen in the office on 10/26/2010. He has had decreased vision in both eyes for about five years. He was told about twenty years ago by a retina specialist that he spent so much time in the sun that he burned out his maculae. He does have two brothers who have dark brown hair and he has always had white hair. He had eye exercises when he was eight or nine because of a lazy eye and his brothers, when he was little, used to call him an albino. He has been diagnosed by dermatologists as having possible albinism but he has been told by other people his skin is just fair. He does smoke a pack of cigarettes a day.
VISUAL ACUITY: Vision OD is 20/70, OS is 20/70. IOP: 20 in both eyes.
SLIT LAMP EXAM: He has retroillumination defects throughout the iris. There is 2+ nuclear sclerosis.
EXTENDED OPHTHALMOSCOPY:
OD: Vertical C/D ratio is 0.5. There is no posterior vitreous separation. There is foveal hypoplasia.
OS: Vertical C/D ratio is 0.5. There is no posterior vitreous separation. There is foveal hypoplasia.
OCT SCAN: There is no foveal dip in either eye with a retention of the normal layers throughout the fovea.
PHOTOGRAPHS: Photos confirm clinical findings.
FLUORESCEIN ANGIOGRAPHY: FA shows no abnormal macular hyperfluorescence. The vascular pattern around the fovea though is typical for oculocutaneous albinism.
IMPRESSION: 1. FOVEAL HYPERPLASIA – BOTH EYES
2. OCULOCUTANEOUS ALBINISM
DISCUSSION: I explained to the patient the retinal findings, including the OCT scan confirming the absence of a foveola and the fluorescein angiogram and photos showing abnormal macular vasculature, all are consistent with his oculocutaneous albinism. Unfortunately, there is nothing to do to or for his eyes at this point, but I did ask him to see you back regularly and I suggested he come here for check in one year
