Macular Laser Scars - Central Retinal Vein Occlusion - Patients Only Eye

80-year-old woman was seen in the office on June 25, 2012. She lost vision in the right eye in 1973. The eye had to be removed, because of end stage glaucoma. It was secondary to an injury and traumatic glaucoma. She has had glaucoma for sometime. She had cataract surgery done along time ago and she is currently on Alphagan, Betimol, and Azopt.
The left eye had a vascular occlusion, probably about five years ago, subsequent to that she had macular laser.
She had peripheral laser. She also had vitrectomies. As far as she can recall about one and half years ago. She
said since then the vision has been poor. She was told at one point that her vision was back to normal and she was disappointed when she went to her son’s police academy graduation that she couldn’t see well. She has a lot of trouble with light and dark adaptation.
VISUAL ACUITY: OD: Prosthesis, OS 20/160. IOP: OS 15. The left eye has 3+ conjunctival injection.
The posterior chamber intraocular lens is in good position with an open capsule.
EXTENDED OPHTHALMOSCOPY:
OS: Vertical C/D ratio is 0.9. There are optociliary shunt vessels on the optic nerve. There is macular and peripheral laser. Unfortunately the macula laser is nearly confluent and there are scars involving the center of the fovea.
SPECTRALIS-SD-OCT SCAN: The OCT scan shows the retina to be atrophic.
FUNDUS PHOTOGRAPHY - AUTO FLUORESCENCE: The auto fluorescence and infra red image shows multiple macular scars.
IMPRESSION:
1. MACULAR AND PERIPHERAL SCARS – LEFT EYE
2. PREVIOUS CENTRAL RETINAL VEIN OCCLUSION – LEFT EYE
3. OPTOCILIARY SHUNT VESSELS - LEFT EYE
4. PREVIOUS VITRECTOMY – LEFT EYE
5. ADVANCED GLAUCOMA – LEFT EYE
6. PROSTHESIS – RIGHT EYE
DISCUSSION: I explained to the patient at the moment we don’t have anything that we can rejuvenate or replace scarred and damaged maculas, but there is a lot of research on stem cells and also on possible neuro regeneration.
