Multiple Evanescent White Dot Syndrome - Atypical Foveal Yellow Spot Right Eye

12-year-old was seen in the office on September 10, 2009. He has had decreased vision in the left eye from reported amblyopia since around the year 2000. Although patching was tried and glasses, his vision in that eye never improved very much. His right eye was doing fine until Tuesday, which is two days ago, when he had sudden severe vision loss. He sees a blurred spot in the middle of the vision in the right eye wherever he looks.
His family history is benign. There is no history on the father’s side or the mother’s side where anybody in the family has any sort of vision loss problems, other than just needing glasses. The father’s parents died young and there is not much of a family tree to refer to. He denies any recent flu or illness.
VISUAL ACUITY: OD 20/60 with eccentric movement, OS 20/200. Pinhole 20/70. IOP: OD 14, OS 10. Color vision was ten out of ten correct in each eye. Amsler grid was normal.
SLIT EXAMINATION: Normal with clear lenses.
EXTENDED OPHTHALMOSCOPY:
OD: Vertical C/D ratio is 0.0. The nerve is a little bit thick looking, but I do not see any clear edema. There is no subretinal fluid or hemorrhages under the fovea. There is a 500 micron yellow spot under the fovea. There are a few white spots clustered around the optic nerve and there is trace vitreous cell.
OS: Vertical C/D ratio is 0.0. There is no posterior vitreous separation. The foveal reflex is mottled.
OCT SCAN: The OCT scan of the right eye does show irregularities at the photoreceptor level. There is some thickening under the fovea. The OCT scan of the left eye shows a normal photoreceptor line scan. Photos confirm clinical findings.
FLUORESCEIN ANGIOGRAPHY: Fluorescein angiography of the right eye shows minimal irregularities of the macula vessels. There are areas of hyperfluorescence corresponding to the white spots in the fundus. The left eye similarly has some irregularities at the edge of the foveal vessels.
