29 y.o. Asian Female reports new floaters and blurry vision started in OD approx two days ago, "cobwebs" and blurry vision are around central vision, no flashes, no cobwebs, "smokey" blurry vision. About 2 days ago she noticed vision change. She noticed spots yesterday. They are like black lines and somtimes like dots right above her line of vision. The left eye is fine. She had a bad headache about 3 weeks ago that lasted about 1 week. She doesn't usually get headaches. She just got a cat that is about 6 months old. She got it from someone who had kittens up in Ruskin (not an official breeder). She does not notice any swollen nodes. She has no fevers. VA OD: cc20/125 NccJ10 OS: cc20/32 NccJ1 IOP: TP OD: 13 OS: 16 OCT Macula: Consistent with Macular Edema. Retinal Elevation Consistent with Subretinal Fluid. Average Central Foveal Thickness 200. Fundus Photos: Findings OD: Blurred nerve margin - no star exudate. Findings. Primary: Cat Scratch Disease OD (Probable). Discussion: Cat Scratch Disease OD (Probable): Her presentation is typical for cat scratch disease. I'm going to obtain blood tests for that and for toxoplasmosis and start her on antibiotics. If the blood tests are negative, then I'll obtain further testing. I suspect they will be positive. Macular Edema OD: The macular edema involves the center of the macula and is likely contributing to the patient’s vision loss.
