82 year old man with no visual complaints and serous retinal detachments in both eyes. He had a trial of Avastin with no benefit. PMHx: Diabetes Type II, No Ocular Complications ("No longer diabetic"). Cancer (prostate). Stroke. Ischemic Heart Disease, Chronic. Congestive Heart Failure. SURGICAL HX: Heart Bypass - Double. Prostate Surgery. VA OD 20/20, OS 20/32 OCT MACULA OD: FINDINGS OD: Retinal Thickening Consistent with Macular Edema (Extrafoveal). Retinal Elevation Consistent with Subretinal Fluid (Peripapillary). Choroidal Thickness 328 mm. Average Central Foveal Thickness 249. OS: FINDINGS OS: Retinal Thickening Consistent with Macular Edema (Extrafoveal). Retinal Elevation Consistent with Subretinal Fluid (Peripapillary). Choroidal Thickness 403 mm. Average Central Foveal Thickness 276. ICG OD: FINDINGS OD: In the late frames of the angiogram, there is hyperfluorescence consistent with hyperpermeability of choriocapillaris as seen in active CSR.
