Last additions - Idiopathic Polypoidal Choroidal Vasculopathy - PDT 83 year old man
Recent vision decline in the left eye. He has been receiving treatment for wet AMD in the left eye from 8/2013 with Avastin and Lucentis. He also had PDT done 2 years ago and seemed to respond better to that treatment. He returns with further vision loss in the left eye. (VA was 20/160)
Medical Hx: Aortic Ectasia (05Jun2012). Allergies, Chronic (2020). Arthritis (2000). Atherosclerosis of Aorta (05Jun2012). Basal Cell Carcinoma-Right Ear (Mar/2016). Chronic Kidney Disease Stage 2 (04Dec2019). Paroxysmal Atrial Fibrillation (2013). Hypercholesterolemia. Hypothyroidism (2013). Gout (Onset: 1991). Colon Polyps (2006). Tortuosity of Aorta (05Jun2012). Influenza Immunization Not Received. Pneumococcal Vaccine Received. Major Depression (Chronic, recurrent) (04Dec2019).
Surgical Hx: Herniorrhaphy (left inguinal) (2008). Tonsillectomy (1948). Prostate Surgery (2005-for BPH). Cardiology Ablation (27Oct2017, for Afib).
Systemic Meds: Allopurinol 100 mg oral tablet, coated PO qday (2003-Gout). Alprazolam, 0.5 mg oral tablet prn (2015-Anxiety). Buspirone hydrochloride 10 MG Oral Tablet (Jan/2022-Anxiety). Levothyroxine sodium 137 MCG Oral Tablet PO qday (2013-Hypothyroidism). Daily Multivitamins - 1 tab PO qday (2000-Dietary Supplement). Diphenhydramine Hydrochloride 25mg tab PO qday (2020-Allergies).
