Derniers ajouts - Macular Fold - Old With Pigment Degeneration 79-year-old woman was seen in the office on 4/5/2011. She has had a long-standing macular fold in the right eye associated with retinal detachment repair in 1970. She has a macular scar in that eye. She has optic atrophy in both eyes from glaucoma. She was recently hospitalized after a fall where she broke her right cheekbone and she had surgery for that October 22, 2010. Since then the vision in the right eye has been poor. VISUAL ACUITY: Vision OD is hand motion, OS is 20/50. IOP: OD 14, OS 19. The posterior chamber intraocular lens is in good position in both eyes with open capsules. EXTENDED OPHTHALMOSCOPY: OD: Vertical C/D ratio is 0.9. The nerve is pale. There is a macular fold. There is scarring under the macula. The retina is attached on a moderate buckle. OS: Vertical C/D ratio is 0.9. The nerve is pale. The macula looks healthy. PHOTOGRAPHS: Photos confirm clinical findings. FLUORESCEIN ANGIOGRAPHY: FA shows hyperfluorescence from the pigment epithelial atrophy centrally where the scar is. There is, however, no evidence of any significant retinal vascular occlusion in either eye and there is good recirculation in both eyes. IMPRESSION: 1. MACULAR SCAR – RIGHT EYE 2. MACULAR FOLD – RIGHT EYE 3. PREVIOUS RETINAL DETACHMENT REPAIR – RIGHT EYE 4. OPTIC ATROPHY FROM GLAUCOMA – BOTH EYES 5. RECENT VISION LOSS AFTER TRAUMA – RIGHT EYE DISCUSSION: I explained to the patient that probably the right eye was somehow affected by the trauma. She had advanced damage to that eye from the glaucoma, and I suspect maybe there is some pressure problem or vascular problem.
