Last additions - Laser Scars - Foveal - Focal Laser - Background Diabetic Retinopathy 74-year-old woman has diabetic retinopathy in both eyes. She has had pan retinal laser in both eyes in 2002 and 2003. She has had focal laser in her right eye done elsewhere in the mid 90’s and unfortunately the laser was fairly close to the center and as time progressed she developed foveal atrophy around the scars. She has noticed her vision declining since she was here last a year ago. She does take Travatan in both eyes at night for glaucoma.
VISUAL ACUITY: OD 20/200, OS 20/40. IOP: OD 21, OS 19. The posterior chamber intraocular lens is in good position in both eyes with open capsules.
EXTENDED OPHTHALMOSCOPY:
OD: Vertical C/D ratio is 0.7. There is pan retinal laser. There is an inactive neovascularization superior and inferior to the macula. There is patchy atrophy, which now involves the foveal center.
OS: Vertical C/D ratio is 0.6. There is pan retinal laser and there is inactive neovascularization in the periphery.
OCT SCAN: The OCT scan shows retinal atrophy in the right eye worse than the left eye with an average central foveal thickness in the right eye of 156 microns and the left eye has 223 microns. The nerve fiber layer scan shows nerve fiber layer thickness in the right eye of 84 microns and the left eye has 92 microns. In March of 2008 the right was 91 and the left was 91 microns.
IMPRESSION:
1. OPTIC ATROPHY – BOTH EYES
2. GLAUCOMA
3. QUIESCENT PROLIFERATIVE DIABETIC RETINOPATHY – BOTH EYES
4. MACULAR SCARRING – RIGHT EYE
DISCUSSION: I explained to the patient the retinas both look stable since she was here last a few years. I asked her to return for a check in a year.
