Most viewed - Stargart's Macular Dystrophy - Severe

32-year-old man carries a diagnosis of Stargardt’s disease with poor vision in both eyes since he was 13. He has not noticed any substantial change since he was here 4 years ago. VISUAL ACUITY: OD 10/200, OS 8/200. IOP: OU 13. SLIT LAMP EXAM: Normal with clear lenses. EXTENDED OPHTHALMOSCOPY: OD: Vertical C/D ratio is 0.1. There is pseudoretinitis pigmentosa with bone spicules throughout the macula. There is also an unusual area of vitreal foveal traction. OS: Vertical C/D ratio is 0.1. There are bone spicules throughout the macula. OCT SCAN: The OCT scan of the right eye shows retinal atrophy with unusual foveal retinoschisis and vitreoretinal traction predominately on the temporal edge of the fovea. The left eye shows diffuse retinal atrophy. Photos confirm clinical findings. IMPRESSION: 1. STARGARDT’S MACULAR DEGENERATION – BOTH EYES 2. MACULAR BONE SPICULES – BOTH EYES 3. NEW VITREORETINAL TRACTION IN THE MACULA – RIGHT EYE DISCUSSION: I explained to the patient the right eye does have an unusual form of vitreomacular traction syndrome. Were his vision subjectively affected by it, it might be worth while considering surgery, but given he has not noticed the difference and also because the risk of surgery in him is high because of his retinal atrophy and because of his probable sensitivity to light exposure, I suggest that we leave the eyes alone.
