45-year-old man was seen in the office on January 6, 2010. He has a yellow spot in the back of the right eye and suggest that he come here for an evaluation. He is in good health. VISUAL ACUITY: OD 20/20, OS 20/20. IOP: OD 16, OS 18. The Amsler grid is normal. Color vision is normal. SLIT EXAMINATION: There is trace nuclear sclerosis in both eyes. EXTENDED OPHTHALMOSCOPY: OD: Vertical C/D ratio is 0.5. There is a 5x5 mm flat, yellow choroidal lesion just inferonasal to the optic nerve, two disc-diameters from the nerve with scalloped edges. It looks like a confluent drusen, but it is not elevated. OS: Vertical C/D ratio is 0.5. There is no posterior vitreous separation. The macula and periphery look healthy. Photos confirm clinical findings. FLUORESCEIN ANGIOGRAPHY: Fluorescein angiography shows some staining of the lesion, but no leakage and the hyperfluorescence mostly washes out in the late frames. ULTRASOUND: The ultrasound shows the lesion to be flat. IMPRESSION: 1. AMELANOTIC CHOROIDAL NEVUS – RIGHT EYE 2. ORBITAL LESION – RIGHT EYE
