Combined Artery and Vein Occlusion377 x angesehenPatient comes in with decreased vision in both eyes. VA 20/200-OD, 20/60-OS. Fundus exam shows great amount of macular edema due to artery and vein occlusions. There is some neovascularization on the optic nerve in the right eye. Patient was treated with Eylea Injection in the left eye today and will return for Eylea Injection in the right eye.

Hyperpigmented Vasoproliferative Tumor on Buckle with New Serous Retinal Detachment 20 years post RD Repair377 x angesehen67 year old female RD repair OU 1999 then severe (20/400) macular pucker OD removed 3 months later (twin sister also had bilateral RD’s with PVR). Then dislocated IOL surgery right eye 9/2012. New vision loss in the right eye with serous retinal detachment. Argon laser to dark tumor led to resolution of subretinal fluid within a month.

At age 15 he had a PPV and laser in the left eye and laser in the right eye. The vision is OD 20/80 PH 20/25, OS 20/25. The left eye has a mild cataract. He had prior testing for coagulopathies which was negative. Testing done for syphillis and TB was negative. Additional laser was done to prevent further bleeding in the right eye.

Four treatments with Avastin she was still 20/100 with edema. With two Eylea treatments she improved to 20/32 and has remained dry and with good vision

She states it is worse in the left. Patient has trouble with distant vision. At most recent visit, 1 years ago, VA was 20/20 OU. Two years ago anterior uveitis x 1 episode with normal labs (HLAB27, FTA, ACE, Lyme). 2017-2020 – Lucentis for DME left eye only.
PMHx: Type I DM since teenager, hyperlipidemia

Ocular Lymphoma - Initially presented like auto-immune retinopathy377 x angesehen43 year old man: initial presentation - The vision in the left eye is blurry and opaque for about 2 weeks.  In the central of the vision, when he covers the right eye, in the center of the left eye there is a film with a light in the middle.  This has never happened before.  He has not been sick for the last few months.  A few weeks ago he saw waves on the floor when looking down but that has gone away.  Since the vision loss started the vision has not changed much.  The central vision is not getting better.  
PMHx – Borderline HTN
VA OD: Dcc20/20
VA OS: Dcc20/40-1 IOP: TP: OD:15 OS:19
1+ NS
No uveitis
Labs normal. One positive antiretinal antibody - Arrestin
VA improved over 2 months to normal. Then the patient had 2 more episodes of self-limited vision loss (no treatment). Then 2 years later he presented with a diagnosis of CNS lymphoma and later developed subretinal deposits (last images)

He did not have EDI OCT, FAF, or ICG. All testing was consistent with Chronic CSR. His immunosuppresives were stopped and he did fine for 3 years.

Multifocal Choroiditis and Subretinal Fibrosis with new Subretinal Neovascular Membrane376 x angesehen26 year old African American Girl Saw me at age 10 (16 years ago) with acute multifocal choroiditis and panuveitis which evolved into subretinal fibrosis. She was initially treated with 3 years of methotrexate. Vision ended up 20/20 in both eyes. Her eyes have been inactive since and she was lost to follow-up.
A week ago she noticed sudden decreased vision in the right eye. Within the past few weeks there are more floaters in the right eye.

Pseudoexfoliation Material Peeling.374 x angesehenPatient comes in for regular eye exam. Last exam was 8-years ago. Complains of decrease in VA. VA was 20/30, right eye and 20/40, left eye. Slit lamp photography shows pseudoexfoliation peeling of the lens within 360-degrees. Pseudoexfoliation ring is present centrally. IOP was normal. Will revisit in 3-months for follow up.

Persistent Subfoveal Fluid Following Retinal Detachment Repair374 x angesehenPatient had pneumatic retinopexy for macula off RD and subfoveal fluid persisted for 12 months but was gone at 18 months. IR images show pockets of fluid after the first week.

Chronic Central Serous Chorioretinopathy374 x angesehen77 year old man was seen at uveitis clinic and tested positive to Anti-retinal antibody against 35 kDa and anti-optic nerve antibody. He did not have EDI OCT, FAF, or ICG. All testing was consistent with Chronic CSR. His immunosuppresives were stopped and he did fine for 3 years.

Hyperpigmented Vasoproliferative Tumor on Buckle with New Serous Retinal Detachment 20 years post RD Repair374 x angesehen67 year old female RD repair OU 1999 then severe (20/400) macular pucker OD removed 3 months later (twin sister also had bilateral RD’s with PVR). Then dislocated IOL surgery right eye 9/2012. New vision loss in the right eye with serous retinal detachment. Argon laser to dark tumor led to resolution of subretinal fluid within a month.

Plaquenil was stopped. The vision in the left eye continued to deteriorate to 3/200. 5'2" 127 lb (6.6 mg/kg body weight) -- Vision in these images is 20/80 OD, 5/200 OS - Vision loss now seems stable.

Congenital Hydrocephalus - 21 year old - Papilledema374 x angesehen21 year old man The patient had a v-p shunt removed in January 2017. He has not had escalating headaches. The shunt was put in in 2015. He was born with hydrocephalus. The shunt was possibly overdraining for 5 years so there were problems with slipped ventricles and low CSR pressure. The pressure was checked today and it was perfect. There is still a bulb so the CSF pressure can be checked. There was eye examination in November showing possible papilledema.

VA 20/200 OD, 20/50 OS. 3 years ago she had a CRVO in the left eye. She is also diabetic for 20 years, has carotid insufficiency and anemia. Left eye shows CME. This was not treated and the vision improved to 20/40 over the next year although mild edema persisted.

Ocular Lymphoma - Initially presented like auto-immune retinopathy374 x angesehen43 year old man: initial presentation - The vision in the left eye is blurry and opaque for about 2 weeks.  In the central of the vision, when he covers the right eye, in the center of the left eye there is a film with a light in the middle.  This has never happened before.  He has not been sick for the last few months.  A few weeks ago he saw waves on the floor when looking down but that has gone away.  Since the vision loss started the vision has not changed much.  The central vision is not getting better.  
PMHx – Borderline HTN
VA OD: Dcc20/20
VA OS: Dcc20/40-1 IOP: TP: OD:15 OS:19
1+ NS
No uveitis
Labs normal. One positive antiretinal antibody - Arrestin
VA improved over 2 months to normal. Then the patient had 2 more episodes of self-limited vision loss (no treatment). Then 2 years later he presented with a diagnosis of CNS lymphoma and later developed subretinal deposits (last images)
