OD stabilized to 20/20 with PRP laser. Now on 6 month checks.

She can see green lights in the night but not in the day. She sees better on a dark background than on a light background. Her mom is genetically visually impaired with Stargardts. Her father has RP. The mother was genetically tested in Boston about 23 years ago. That was when the gene was first being isolated. (The mother has seven brothers and sisters and four have stargardts and they are legally blind. Also one cousin has it.)

The patient started noticing dimming of the vision in November of 2020.Â He had rapidly worsening cataracts. He had the cataract surgery done in both eyes in April of 2021.Â The vision did not improve with the cataract surgery.Â On June 1 of 2021 he had immunotherapy for his renal cell carcinoma and then developed severe vision loss.Â The patient noticed that one of the commercials for nivolumab (checkpoint inhibitor targeting programmed death receptor-1) said it may cause vision loss.Â He recalled that he started the nivolumab in October of 2020 and had vision loss starting then.Â Â A retinal evaluation showed subretinal fluid and vision loss. Â The peripheral vision and central vision were pretty good.Â The vision did seem to get better off the nivolumab.Â Then when the patient returned to Florida in September and now the vision is getting worse.Â The patient did not have any treatments in either eye.Â The patient is now off the oncology treatment.Â He was then started on a TKI cabometex for the cancer which was dosed in mid June through mid July just for a month.Â It was stopped because of a skin rash and reactions.Â Â

VA OS: Dcc20/63 PHNI NccJ7 He has been off the checkpoint inhibitor for a year.

Retinal detachment with choroidal effusions and RPE tear292 views83 year old man with chronic glaucoma presented with 10 days of vision loss in the left eye and a retinal detachment. He also has choroidal effusions and and RPE tear. He had a history of bladder cancer in remission. I did a biopsy of the choroidal lesions at the time of surgery which showed no cancer. Initial vision was HM and final vision was 20/80. The choroidal effusions may have occurred because of his age and prior history of glaucoma.

Cilioretinal Artery Occlusion - non central292 views81 year old female She noticed sudden vision loss in the right eye just before Christmas while watching television.Â This was about one week ago.Â She noticed when she closed her left eye and looked out of her right eye there was a darkness in the vision.Â The left eye is OK.Â She has no history of stroke and no neurological problems.Â She had a few headaches recently but they went away.Â Â
PMHx: HTN, Breast Cancer
Meds: HCTZ, Atorvastatin, Atenolol, Anastrozole
VA 20/80 OD, 20/32 OS

VA improved to 20/40 in one month in the involved eye

Severe Syphilitic Placoid Chorioretinitis292 views68 year old man 3 months ago the vision started fading in both eyes. Just sitting here he can see some light out of his right eye. The patient denies any recent illness or travel.
VA: OD: LP, OS: 4/200
IOP: 9, 13
Anterior segment: 1+ cell/ 2+ flare OU and 2+ NS and anterior vitreous cells
Labs Reactive RPR 1:256, FTA-ABS - reactive - Patient was admitted to hospital for IV PCN and recovered vision almost completely in the left eye and some in the right eye.

Sticklers syndrome with Pierre Robin Sequence292 views9 year old child with father who had retinal detachment at age 10. History of cleft palate repair and teeth removal. VA 20/40 OU. Images show perivascular RPE and retinal changes especially nasally

Papilledema Lateral Sinus Thrombosis292 views35 year old female Noticed blurred vision when waking up in the morning. Also, vision blacks out for a few seconds when she gets up quickly. About 3 months ago she had a history of intermittent vomiting.

MRI normal, MRA showed lateral sinus thrombosis. Lupus anticoagulant was positive

Severe diabetic tractional retinal detachment one year following diagnosis of type II Diabetes Mellitus292 views44 year old man The vision in the left eye started to decline about 4 days ago.Â Prior to that the left eye was the better eye.Â He was diagnosed with diabetes a year ago.Â

Choroidal Hemorrhage following intraocular lens exchange surgery292 views68 year old man Patient is 1 week post IOL exchange with sutured Akreos PCIOL with PST Kenalog. IOP was 12 on POD 1 and 21 today (POD 7). Preop the IOP was 17.
He has a history of retinal detachment repair 12 years ago with PPV, Buckle, Laser and Gas.

VA OD: 20/200, OS: 20/50. IOP 21, 19

Birdshot chorioretinitis with macular edema - positive HLAA29292 views67 year old female with 'vitreous hemorrhage' in the right eye for a year. Extensive uveitis work up done but now with HLA A29. HLA A29 was positive. She has not responded to kenalog injections (posterior) and will be started on systemic therapy soon

VA OD: Dcc20/25-2 OS: DccCF 1ft
IOP: TP: OD:13 OS:12
Patient's right eye was treated with Avastin and was dry with one shot.

Massive subretinal hemorrhage right eye - polypoidal choroidal vasculopathy292 viewsPatient had sudden vision loss right eye. Had vitrectomy. The left eye had IPCV not responsive to avastin but he did respond to PDT. See album notes for more detail.
