OD 20/40, Pinhole 20/32. OS 20/160, Pinhole 20/50. He has band keratopathy sparing the visual axis in both eyes. ICG shows angioid streaks both eye.

Retinitis Pigmentosa - Autosomal Dominant - Vision Loss at Young Age - Infraredvista 564 veces 54-year-old woman has autosomal dominant retinitis pigmentosa. Her mother and grandmother both had it and she has one son who has it and one daughter who doesn’t. She also has a sister with it. She lost vision since she was younger. She had cataract surgery done in January and February of this year. She has been seeing unusual flashing lights throughout the periphery and central vision for about the last year or two.

Subsequent to that his vision was not quite right in the right eye and then he noticed, starting about three weeks ago, substantial change in the vision. In the morning he sees what he calls a small gray circle with a ring of fire around it and then subsequent to that the vision becomes more normal but not quite right. His vision in the left eye is fine. Vision OD is 20/50, OS is 20/16.

He has a macular scar in the left eye from choroidal neovasculari-zation that was treated with photodynamic laser 5-8 years ago. Myopic Macular Degeneration - Staphyloma - Recent Vision Loss Right Eye - Possible CNVMvista 563 veces83-year-old woman is a high myope with myopic macular degeneration. She had a retinal detachment repair in the right eye in 1968. I saw her in June of 2003. She had posterior colobomas and areas of pigment epithelial atrophy. She has noticed over the last six weeks substantial declining vision in the right eye. Prior to that, the vision in the two eyes was about the same. She uses Xalatan in the left eye and Alphagan in both eyes.

VISUAL ACUITY: Vision OD is 20/160, OS is 20/40

vista 562 veces33-year-old man with a history of decreased in the right eye. He had blurring in the eye about a year ago. He reports that it improved spontaneously until it was “nearly resolved”. He said he had some persistent distortion, but that over the past couple of weeks he has noticed this dark spot recurring in the right eye, the left eye is unaffected. He does not have any flashes, floaters or change in his peripheral visual acuity. 20/80 OD, 20/16 OS. No improvement with pinhole. IOP: 12 OD, 11 OS.

POSTERIOR SEGMENT EXAMINATION: Shows a clear view with no PVD in either eye. C/D ratio is 0.2 OU. Large retinal vessels are unremarkable. The macula of the right eye has a brown choroidal lesion with pigment overlying the surface and orange spots along its lateral surface. The left macula is normal.
B-SCAN: B-scan ecography of the ultrasound of the right eye shows a mass about 5 mm high with a moderate to low internal reflectivity on A-scan echogram. There is serous fluid inferiorly on the echogram

PDR - Mild Vitreous Hemorrhage and Good Visionvista 561 veces58-year-old woman has diabetic retinopathy in both eyes with neovascularization of the optic nerve, worse in the right eye than the left eye. She has had panretinal laser in both eyes and vitreous hemorrhage in the left eye. Her vision, since I saw her last in June, is about the same.

VISUAL ACUITY: Vision OD is 20/25, OS is 20/20.

Old Branch Retinal Vein Occlusion- Severe CME - Pucker - Diabetic Early FAvista 561 veces61-year old man has diabetic retinopathy in the right eye. He also had a macular pucker and macular edema. I did a vitrectomy on September 5th. His vision was initially improving. His vision in the right eye seems to be a little more foggy.

Birdshot Chorioretinitis - Retinal Vasculitis - HLA A29 Positive vista 561 veces64-year-old woman three or four year’s fluctuations in her vision. She mostly notices them in bright light her vision glares out and she has difficulty seeing. PMHx: malaria in 1948 that was treated. She was told at one point she could possibly have had Familial Mediterranean fever.

She does intermittently get rashes around her arms. She has arthritis in her knees and ankles.

Choriodal Neovascular Membrane - Subfoveal - Pseudo-Retinitis Pigmentosa - 50 years post Retinal Detachment Repairvista 561 veces86-year-old with a history of decreased vision in the right eye. The patient reports she has lost vision in the right eye over the past two months or so. She reports the vision was always compromised in that eye since retinal detachment surgery 50 years ago with a scleral buckler. Vision dropped from 20/50 to 20/200 right eye in 2 months.
